Banked memory is no longer needed.
- (pATI->BankInfo.BankSize = 0) in all cases, cull pATI->BankInfo
- only keep the minimal pATIHW.SetBank interface for save/restore
- clean ATISwap() a little, (NewHW.crtc != ATI_CRTC_VGA)
- (UseSmallApertures == TRUE) <=> pATI->VGAAdapter
