Refactor memory map / unmap functions.
MGAMapMem and MGAMapMemFBDev were refactored into a single function.
This happens to fix a bug triggered in software XAA paths because
MGAMapMem did not set FbStart. MGAUnmapMem and MGAUnmapMemFBDev were
also refactored into a single function.
