Replace VM_{MAX,MIN}_USER_ADDRESS with VM_{MAX,MIN}_ADDRESS.
Change a while() loop with a clearer for() loop.
Fix pmap_page_protect to correctly handle the VM_PROT_READ|VM_PROT_WRITE
case.
