Simplify new LUN scanning logic, add diagnostic messages for all
instances of bad LUNs and add SCSIFORCELUN_BUSES and
SCSIFORCELUN_TARGETS options.
ok miod@ deraadt@. Tested in the Marco Peereboom torture chamber.
