Yet another batch of -Thtml polishing from kristaps@:
In particular, use <SMALL> for .SM and <CODE> for .Dl.
Use <B> for bold and <I> for italic in general.
Also call this mandoc 1.10.8 now, as it is functionally equivalent,
even though one one set of refactoring patches has not been merged
yet because it conflicts with our tbl(1) handling.
