introduce xcalloc() and xasprintf() failure-checked allocations functions
and use them throughout openssh
xcalloc is particularly important because malloc(nmemb * size) is a
dangerous idiom (subject to integer overflow) and it is time for it to
die
feedback and ok deraadt@
