entries to decide if the IFF_ALLMULTI flag should be set, check if there
is at least one real range between them.
This should not change the behavior of any driver but if you encounter
any problem, feel free to revert the offending chunk and ping me about
it.

- the am7990_get() - now lance_get() - is unchanged.
- the interrupt acknowledge logic is unchanged, and will disable interrupts,
then acknowledge all interrupt conditions.
Add ILACC (79900) support (from NetBSD).
Both LANCE (am7990.c) and ILACC (am79900.c) code share as much common code
(lance.c) as possible. This affects all le(4) attachments, but the changes
are mostly mechanical, to split am7990-specific parts from lance-agnostic
parts.
Compile tested on all affected platforms. Tested on alpha, hp300, luna88k,
mvme88k, sparc, sparc64 and vax.

a physical address [more precisely, something suitable to pass to pmap_enter()'sphysical address argument].
This allows MI drivers to implement mmap() routines without having to know
about the pmap_phys_address() implementation and #ifdef obfuscation.

to 18 bytes by removing the extra_bytes field. sbic.c was the only
consumer, and that was in a DEBUG message.
18 bytes is the minimum length mandated by the SCSI spec. Anything
more is vendor/device specific information.
This fixes a usb key that yuo@ had, and probably others.
Any device that decides to pay attention to extra sense data must
allocate its own space.

max-baud-rate hint. Adjust TTYHOG (the nearly full logic) to this new
situation. The larger buffers are required by the very high speed
KDDI devices in Japan (CF com, or USB ucom) so those are the only two
drivers which currently ask for a larger buffer size.

void. Use XS_NO_CCB error in the scsi command (xs) to report the
NO_CCB condition. Eliminates all SUCCESSFULLY_QUEUED and COMPLETE
confusion and untangles the midlayer from the adapter a bit more.

There may be some compile issues on little used (i.e. I don't have
any) drivers but the change is mechanical and thus easy to remedy.

TRY_AGAIN_LATER uses with equivalent NO_CCB. Eliminates confusion
between the two as was always intended. buf I/O's that can't be
started get pushed back onto the front of the queue and retried.
Others get sent back to originator as failures. No more epi-cycle
looping inside the SCSI midlayer hoping the problem goes away.
Various testers, no objection from miod@ as vs(4) was tested by

remove it from any occurences where both are used, except one for kqueue itself
