When Joerg Wunsch copied most of the text in this file from sysexits.h,

he neglected to copy the Copyright notice as well; so restore the Copyright
notice as it was originally set up by Keith Bostic.

While here,
* correct the BSD release where this file first appeared,
* mention the original use case and author,
* kill low-level roff(7) tinkering, and
* clarify what exactly Joerg copied.
