Significant improvements to -Thtml by kristaps@:
Use less <DIV>, use more <H1>, <H2>, <P>, <BR>, <PRE>, <UL>, <OL>, <DL> etc. Triggered by input from Will Backman. Remove CSS2 note in mandoc.1, which is no longer true.
